The Health Reset Program is exclusive to eligible St Lukes members.

Do you need a reset to manage a health condition?

The St Lukes Health Reset Program, in partnership with Salveo Healthcare, helps you take control of your health in a way that fits your life. This program gives you support as you work toward your health goals when and where it works best for you. Your plan is made with you, and you’ll get regular check-ins from our nurses and allied health team to help you along the way.

Who is the Health Reset Program for?

The Health Reset Program is for St Lukes members living with a long-term health condition and would like to take charge of their health. The program gives you the tools, support, and confidence to make choices that work for you, so you can lead your health journey, your way.

How does it work?

The program runs for up to 12 weeks and you can do it from home (either in person or online). You’ll get useful tools and support from our nurses and allied health team to help you reach your health goals.

The program is all about you – your goals, your choices, and your health journey. We’ll help you make a simple plan, so you feel informed, confident, and in control.

It’s flexible and it’s free for eligible St Lukes members.

Am I eligible?

You may be able to join this program if:

  • Aged 18 years old and over
  • Diagnosed with one of more chronic conditions
  • A St Lukes member for a minimum of 1 year with hospital cover (mandatory)
  • Able to use mobile phone or tablet apps (not essential)

Information for Referrers

Who can refer to this program?

Anyone can make a referral to the program. You might be:

  • A medical or health professional
  • A carer
  • Or a family member or friend

Eligibility criteria

You may refer someone to join this program if they are:

  • Aged 18 years old and over
  • Diagnosed with one of more chronic conditions
  • A St Lukes member for a minimum of 1 year with hospital cover (mandatory)
  • Able to use mobile phone or tablet apps (not essential)

How to make a referral

To refer a person into the Health Reset Program, follow these simple steps:

  1. Download and complete this referral form.
  2. To help us understand whether the program is right for the participant, we’ll need a current health summary or care plan as part of the referral.
    • If you are a GP, please include this with the referral form.
    • For non-GP referrals, you do not need to include this with your referral form. We will follow up to obtain it prior to acceptance into the program.
  3. Email the completed referral to general@salveohealth.com.au.

Once a referral is received, St Lukes will review it. A St Lukes Care Coordinator will then contact the participant to check that:

  • the program is suitable for their health needs, and
  • they have the right level of cover.

Submitting a referral doesn’t guarantee acceptance into the program. All referrals are subject to St Lukes’ Terms and Conditions.
